Details and requirements
- Register at www.covid19vaccine.safemakati.com
- Choose type: Makati Resident, Non-voter or Non-Makati Resident
- For those questions that you have no answer, type 0000 if asking for a number or N/A. Don't leave it blank if there is an (*) asterisk.
- Once you are able to submit, it will give you a confirmation number. Take screen shot and save it.
- Brand of vaccine: Pfizer
- Child must be accompanied by a Parent/Guardian.
- If with Parent, bring any valid Gov't issued ID.
- If with Guardian, bring any of the following: Affidavit of Guardianship, Affidavit of Kinship, School ID records showing Guardian's name
- Bring copy of Birth Certificate of the child or any document proving age and relationship to the Parent or Guardian.
- Bring Medical Clearance for those who have comorbidities.
